[Sigia-l] How do you make HTML wireframes

When creating wireframes and reviewing with clients, I have had
instances where the first draft was a hit and we were ready to move on
to design.
There have also been a time or two were anything my team or I did was
not meeting the clients needs/vision and resulted in as many as 8 to
10 iterations of wireframes before me got to a fine-tunning level.
 
The ladder might be a very extreme instance but it made me think about
the revisions process when utilizing HTML.
 
For those of you who do create HTML wireframes, are you employing a
"code library" to easily produce common components? What is your
general process? Has turn-around time ever been an issue? (I know at
least for me that a wireframe in Visio that takes about 10 minutes to
complete would probably take around 20 minutes in HTML- but I also
code by hand- not a fan of WYSIWYG)
 
In addition, are there set standards within your organization, or at
least agreed upon procedure between you and the development team, on
code formats so these templates can be utilized for the end-product?
