I've also found this happening whenever I use a large monitor with a
high resolution in windows. I often don't maximise applications when
working. Similar issue, I guess it's a little more pronounced in dual
monitor setups.

I think this deficiency in layering is one of the shortcomings of the
'desktop' metaphor. On a real desktop one would be less likely to miss
which document is on top. There is also less of a chance that I would
work with 5 documents on top of each other (I would move all others to
the side).

But since I can't remember the last time I actually sat and worked on
a desk the metaphor itself will have to 'serve' this need. Maybe a
taskbar that follows the mouse? or maybe the gradual fading of
non-active windows as in mac-osX? How about a visual display of layers
like photoshop? I don't know the exact answer but I am sure there must
be one out there.

Or, there might just be a revolution in display technology and the
problem will disappear or become less acute.
