I wouldn't say unnoticed, just accepted and almost expected in a frightening
sort of way...e.g:

The list goes on and on. Large scale projects such as these have a tendency
to turn into lumbering juggernauts and it is very difficult to turn them
around.

Indeed. The most productive use of time is to get focus and direction from
high level requirements and then iterate through the details.
