I agree wholeheartedly. They will never learn unless they are hold
accountable. What I pointed out was the fact that they are NOT held
accountable. In this context, it doesn't make sense trying to look for
people to blame, because the blame will be diverted from them to the CM tool
and/or vendor.

The well-meaning incompetent, inexperienced & unqualified ones should be
held responsible, but making that real and seeing that happening within
organizations is a different subject altogether.

Back to the subject: I think there is a much higher chance of the problem
being the hands of purchasers than in the hands of vendors. And definitely
not the CM tool itself. As the name says, they are just that, tools. "tools,
can't do the human job of adequating needs and meeting requirements."

Well, going a step forward, without placing the "blame" squarely where it
belongs, the individuals (and the organization as a whole) don't learn.

and thus unqualified for their positions? Why give them a pass?

The organization/company (the 'injured' party here) should look for the
target of the blame. For its own good. Those that don't do an adequate
post-mortem (which involves blame apportioning) will fail to learn.
