Billing may be based on time, estimate, or perceived value.

   * When I start working on your project, I'll keep track of how 
much of my (valuable) time I spend.  That will be your bill.

   * I expect your project will take me 300 hours, but it may be 
more.  I'll quote you a fixed price based on 400 hours at my highest 
rate (but I'll not give you that computation -- just the number).

   * The result I'll give you will make a significant impact on your 
business -- clearly a $50,000 proposal is in order.  In fact, I don't 
think any body will do it for you for that amount, so I'll quote you 
$60,000.  I might make a killing.

I'm stuck in #1, which I fully understand and know how to manage.  My 
clients trust me to deal honestly and to not abuse the 
process.  Sometimes I'll consult with them before spending lots of 
time on a side issue, so they remain somewhat in control.  But all 
three models make sense.
