In the U.S. this is not a problem, in fact, the bigger the screw up scale,
the better. And I don't mean that facetiously.

Just one example: I witnessed a fairly high-level manager promoted 10 days
after the group she was directly responsible for 'lost' $10 million through
sheer technical incompetence. I was flabbergasted, others told me, however,
that it wouldn't amount to a rounding error in that gigantic bank's balance
sheet.
