I usually have interaction flowcharts (boxes and arrows) on the left side
running down and call-outs (for explanation, comments, questions, etc)
placed on the right attached to specific items. Boxes contain simple
descriptions; details, attributes, exceptions, ranges, etc go into the
call-outs on the right.
These are presented in PowerPoint slides and later sent to stakeholders in
one, long, continuous PDF. The latter place in-line digital comments and the
annotated PDFs get circulated.
I don't do much wireframing (often skipping to the prototype as early as
possible) but wireframes get the same treatment.
Of course, you have the option of making individual PPT items clickable to
give the appearance of interactivity, and I do that if there's no prototype
ready yet.
