When I was at IKON we used a version of IBM's 12 questions for our 
quarterly user satisfaction surveys.  We also used a series of focus 
groups and individual interviews to augment our results.  Most actionable 
items came from a careful analysis of the open ended questions in the 
survey (the negative and positive aspects of the site) and the feedback 
gathered in the focus groups.

I've found Perlman's article on computer administration of usability
questionnaires a useful place to start. The questions are generic and
mostly use Likert scales but they are all from well designed question
sets.

Hi, 

I have an opportunity to submit a survey to an organization who we're 
redesigning an Intranet for. Was wondering if any of you have had 
experience with this or would like to suggest some questions that we 
should definitely ask. As far as I know we are able to do any type of 
question including open ended, multiple choice and matrix. My goal is to

get actionable feedback. Will publish complete list of questions to
list.
