Actually just thinking about this (cigarettes may harm me in many ways, but
cigarette-breaks are the mother of invention) that focusing could be done
for those who don't have pop-ups available for lack of javascript by
producing the link in javascript, hence producing no link when js isn't
available.

Those who have *severe* difficulties with pop-ups will often kill js for
that very reason, so their needs are dealt with in this way, and a warning
in the alt and title should adequately cover those with problems with
pop-ups who don't kill js.
