we use murray hill center
<http://www.murrayhillcenter.com/nyc/nyc_facility.html> at park ave south
and 26th -- their rooms are designed for focus group (large table with
10-14 chairs around it) but we use them for individual user
interviews. we're really happy with their facility and have been renting
and using it for usability research for 5 years.

bob granito of interactive video production
<http://www.interactivevideo.com/home.htm> provides picture in picture
video -- or you can bring your own video equipment or rent a portable
'usability lab' from <http://www.usabilitylabs.com/> or
<http://www.neenanpartners.com/> or <http://www.ovostudios.com>

we've used focus pointe in philadelphia -- they also have a space in nyc:
http://www.focuspointe.net/manhattan.html

we've used schlesinger in l.a. -- and they also have space in nyc:
http://www.schlesingerassociates.com/ny.html

here's one that we found on the web but never used:
