Peter,
I had begun experiencing similar problems I did a couple of things that seem
to have alleviated most of the problems.
1. This is my new mouse:

weird to get used to, for the first few days I kept dragging it around the
desk. (Note: I work for OD so feel free to shop around)
2. I began switching mousing hands - the mouse above makes it easy to switch
from right to left with a little practice. As an added incentive; I did
read a study that suggested that using your non-dominant hand to do normal
daily activities increases neural pathways - I guess you can never have too
may neural pathways ;)
3. I also raised my chair and lowered my keyboard so that when I sat with my
feet flat on the ground I was not slouching and was holding my wrists off
the desk.
4. If you find that you cannot retrain yourself to keep your wrists off the
desk you can use a keyboard cushion - the gel ones are best and don't wear
as badly as the foam.
You may also want to pick up an old copy of the game Key Commando - fun game
for learning keyboard short cuts for a number of popular applications - I
don't believe that it is on the market any more but I am sure there is a
copy on Kazaa somewhere ;o)
