Most recently I've used www.surveymonkey.com.  It's easy to use and provides many options for survey design and data analysis. They also provide a free basic subscription.

Subject: [Siguse-l] Looking for survey software

I'm looking for a good (and not too expensive) survey package to use in a
couple of upcoming information use research projects.  One will be a survey
sent out via email and the other will be a more traditional, mail-based
questionnaire.  Ideally, I'd like to find a program that will support
both--and will include some rudimentary data analysis functions as well.
Does anybody have any experiences/recommendations of survey packages to
share?
