Emphasis is commonly placed on impact factor--ie, the measure of how frequently reports in a given journal are cited. Proponents and opponents continue to have heated discussions on this topic. The journals with the highest impact factors in each field are an attractive target for many workers. Aficionados interpret the study of impact factors as a relevant assessment of the state of affairs in the different branches of medicine. Sometimes the results of such comparisons are, however, surprising, such as the difference between immunology journals (maximum impact factor 37?796) and those for rheumatology (6167?0). The development of immunology has meant that rheumatology might be seen as a practical form of immunology, and the difference in bibliometric weighting is therefore not easily understood. Many complex factors play a part in the citation intensity of a given journal, such as quality, circulation, and accessibility, and much attention has been devoted to these factors. One obvious factor has, however, been missed: the opportunity for citation. To assess this feature at random, we studied an ISI Journal Citation Report (JCR) more closely. We investigated the impact factors for the top journals in the different specialties, as defined by the ISI JCR, 1997. The number of journals in each specialty correlated significantly with the top impact factor achieved (Spearman's rank correlation r=0?7692 [95% CI 0?5211-0?8974], p<0?0001). When we assessed only dermatology journals, we noted a similar correlation for top and mean impact factors for the period 1991-2000. This finding suggests that the number of journals per specialty is a strong confounding factor in the determination of traditional or basic impact factors. The difference in the number of journals (117 vs 18) might, therefore, alter basic impact factors, for example for immunology and rheumatology disproportionately. To assess this effect, we calculated weighted impact factors by dividing basic impact factors by the number of journals in the specialty. Differences between specialties are not fully accounted for, but the weighted impact factors of clinically related specialties seem more similar for basic impact factors. Weighted impact factors, therefore, seem to offer a more appropriate measure for comparisons than do basic impact factors. Top journals should welcome and encourage new publishers, since more journals means a higher top basic impact factor. When we evaluate all universities in China, rather than just one type of university (medical university, for instance), we shuld not ignore either database. Once they are used in a comprehensive indicator, their complementary sizes and disciplinary structures produce an unexpectedly good result. China is the largest country in the world in terms of population, but for many reasons (the language barrier in particular), at present the amount of paper published by Chinese scientists and covered by SCI (1998) ranks only twelfth in the world. Most papers are published in domestic journals. In general, SCI covered papers are of higher quality and of stronger impact. METHODS Papers published over 1991-1999 which were recorder in the MedLine database (CD-ROM) were selected by using the key words much less than Spain much greater than or much less than Espana much greater than in the address of author field and much less than HIV much greater than, much less than AIDS much greater than, much less than VIH much greater than or much less than sida much greater than in any field. Papers were analyzed according to publication journal specialty of authors, subjecte matter of the paper, institution, center of origin and geographical distribution. RESULTS: A total of 2,065 papers were studied. Their number doubled, from 148 in 1991 to 295 in 1999 (r = 0.917; p < 0.0001). The number of papers published in foreign journals trebled, from 51 to 182 (r = 0.933; p < 0.001). The journal with the highest number of papers published was MEDICINA CLINICA 241 (11.7%). Although the main topic was the clinical manifestations of HIV infection (64.6%), the proportion of papers on this subject decreased from 71.4% in 1991-93 to 58.3% in 1997-99 (p < 0.001). On the other hand, the percentage of articles on antiretroviral therapy increased from 3.6% in 1991-93 to 12.4% in 1997-99 (p < 0.001). The greatest number of contributions came from hospitals (79.8% of papers), followed by universities (9.5%). Centers with the highest output were the Hospital Clinic (Barcelona) (5.7%) and the Hospital Carlos III (Madrid) (5.5%). The Autonomous Communities of Madrid (33.1%) and Catalonia (25.3%) concentrated more than half of the total output, followed by Andalusia (11.5%) and the Valencian Community (8.1%). CONCLUSIONS: Spanish output on HIV infection/AIDS research grew during the 1991-1999 period. There was a remarkable increase in the papers published in foreign journals. Moreover, topics changed according to the epidemiological changes observed in HIV infection during the past decade. This article attempts to provide such evidence through an evaluation of Ingwersen's (1998) proposed external Web Impact Factor (WIF) for the original use of the Web: the interlinking of academic research. In particular, it studies the case of the relationship between academic hyperlinks and research activity for universities in Britain, a country chosen for its variety of institutions and the existence of an official government rating exercise for research. After reviewing the numerous reasons why link counts may be unreliable, it demonstrates that four different WIFs do, in fact, correlate with the conventional academic research measures. The WIF delivering the greatest correlation with research rankings was the ratio of Web pages with links pointing at research-based pages to faculty numbers. The scarcity of links to electronic academic papers in the data set suggests that, in contrast to citation analysis, this WIF is measuring the reputations of universities and their scholars, rather than the quality of their publications. However, fundamental questions remain as to whether or not patent statistics represent the real state of innovation. As Griliches pointed out, substantial questions involve: What aspects of economic activities do patent statistics actually capture? And, what would we like them to measure? He pointed out that these statistics can be a mirage appearing to provide a great number of objective and reliable proxies for innovation. This paper aims to address some of these questions by making a comparative evaluation of the representability of patent statistics in four levels of the innovation process, using as examples research and development (R&D) in Japan's printer and photovoltaic solar cell (PV) industries over the last two decades. The correlation varies with the sub-field: it is almost zero in nursing research but is moderate in more "scientific" subfields such as multiple sclerosis research, characterised by many authors per paper and appreciable foreign co-authorship. If research evaluation is to be based on journal-specific indicators, then these must cover different aspects of the process whereby research impacts on other researchers and on healthcare improvement. In contrast, nearly one half of the authors have been cited only once. Similarly, the top 1% (10%) of articles/books received 22% (56%) of the total number of citations. These results indicate that a few prominent researchers dominate citation in the leading finance journals. Such grading is needed both for funding and resource allocation purposes and for personnel decisions. In grading journals, objective information, such as the impact factors (the most commonly used journal citation information provided by Journal Citation Reports) and/or subjective information, such as experts' judgments about the journals, are used. Grading of journals is typically done by a committee. Most of the existing approaches only consider either the subjective or the objective information. While such approaches work in many cases, they may not work in many others. Complaints about politics, inconsistencies, and unfairness are common in many situations. This paper presents a methodology that integrates both types of information, providing a comprehensive method for assessing the quality of journals. The method employs a fuzzy set approach that also deals with the imprecise and missing information frequently inherited in the evaluation process. The proposed evaluation model is implemented in a Web-based system. A pilot evaluation study indicated that participants clearly preferred the system and its major features. Using the Arts and Humanities Citation Index database, the author's institutional affiliations are identified and examined with the intention of verifying a community of Annaliste historians publishing research within the parameters of Annaliste historiography and intellectual concerns. A discussion of domains of knowledge and disciplinarities is linked to intellectual orientation. This study indicates that these historians are predominately French with a significant minority representing an international community. Substantive knowledge of Annaliste historical research, methodologies, and intellectual orientation animates institutional affiliation. Finally, the authors suggest that identifying any major "school" of historiographical innovation can be accomplished through use of bibliometrical analysis. Design: All articles published in 1983, 1988, 1993, and 1998 in 5 US surgical journals and 1 British surgical journal were reviewed and characterized. Main Outcome Measures: Absolute numbers and proportions of national and international articles published in each journal. Results: Articles reviewed included 4868 in US journals and 1380 in the British journal. The total number of US journal articles decreased by 15.1%. The total number of British journal articles increased by 58.9%. The percentage of national articles decreased from 87.5% to 68.8% in US journals (P<.001) and constituted the minority of freely submitted articles in 1998 in 3 of 5 US journals. The percentage of national articles also decreased from 74.8% to 47.1% in the British journal (P<001). Articles by European and Asian authors showed the most striking increases in all journals. The percentage of basic research articles declined in US journals from 23.3% to 17.9% (P=.001) owing to a 14.9% decline in national basic research articles. The percentage of clinical randomized studies increased from 2.2% to 4.1% (P<.008), but the increase was attributable to international articles. Government funding alone decreased from 13.6% to 11.2%, and government plus another source of funding decreased from 19.2% to 16.7% for national articles in US journals. Conclusions: Internationalization of the highly rated British and the 5 highest rated US general surgical journals has occurred. The decrease in the number of national articles in the US journals has been accompanied by significant decreases in government funding and basic research articles and a static output of clinical randomized studies from North America. The loss of the British Empire may have benefited the BJS which like Britain has had to look increasingly to Europe and beyond and not just to the English-speaking world. This may explain the greater reduction, proportionally, in national articles in the United Kingdom compared with the United States. The increase in European publications reflects a policy change by the BJS to encourage greater cooperation with European surgical societies, providing a wider readership for their members. Thus, during a 30 year period between 1969 and 1999, the number of United Kingdom-based national publications in the BJS decreased from 82% to 47%, whereas the number of European articles increased from 2% to 28% during the same period. There was no discernible trend in publications from the rest of the world. The US journals seem to have attracted a greater contribution from Asia than from Europe. In his review of 9 general surgical jurnals (including the BJS and 3 of the U.S. journals cited in the aticle by Tompkins and co-authors), the editor of The Lancet, in 1996, (1)compared surgical research to a comic opera, with its emphasis on case series rather than randomized trials. Tompkins and colleagues note a decrease in basic research in all publications but a dearth of randomized trials in the BJS between 1988 and 1998. This finding is at odds with the data reported in a leading article in the BJS which note an increase from 3 to 13 randomized trials per year between 1989 and 1999(2). The authors rightly identify the impact of external factors on research, such as the pressure of managed care on academic and clinical departments. In the United Kingdom, the research assessment exercise places greater emphasis on the impact factor of publications submitted for grant applications. This has led to a shift from clinical to molecular research with subsequent publication in scientific journals with a higher impact factor. This may explain the relative reduction in national research articles. The implication for all surgical journals is clear: globalization is here to stay. The differene in teh internationalization of general surgical journals between the US journals and BJS is marked and sould sound alarm bells among the editorial boards of these journals.
