Tom,

The gov.uk site that others have mentioned is a great example of a
user-focused website.

In the US, I know the GSA provides a site for federal agencies:
http://www.howto.gov/web-content. The site covers many aspects of digital
media, including a section on best practices for web content and user
experience. They have a program called First Fridays that includes some
before and after examples of projects they've done:
http://www.howto.gov/web-content/usability/first-fridays

I also know that some federal agencies, including CDC.gov, use ASCI and
other measurement tools to measure customer satisfaction with their sites.
