I agree wholeheartedly. In my experience it is quite obvious that the visual design of a process form has less impact than having the appropriate questions to begin with, but our client is pretty interested in exploring this endless personalization of the interface design based on segmentation.
I was just looking to see if there was any research that supported my opinion, but didn't want to bias my original question to you :)

I would have to agree with everything Juan said.
When people are presented with a form, they want to complete it and
get on with their life as quickly and easily as possible. I don't
have any references to hand, but I'm sure I've read research that
also shows users they are heavily "field focused" and often won't
notice other things (which includes customisation). This has
certainly been my experience from user testing.
