for me it would depend on the cognitive processes of your users.

Is the default behaviour [OK] or is it [Cancel] ?  ... If default is OK then put it first (thinking left-to-right text cultures)

If the culture is right-to-left though, then the default might have to go on the right.

web apps are further complicated by the back button being effectively
[cancel], and you want to avoid the html spec 'cancel' button because all it
does is clear the form (and all data entry so far)
