There was a presentation on this at WebDD earlier this month:

Telerik arrange testing sessions with people by phone and
they capture what's on the user's screen, including pointer
movement. Stakeholders and developers are in the room to
hear the user. They welcome interruptions to the user
during testing as "that's what happens in real life."
The testing package includes instructions so that everyone
gets the same information. They only test five people
(because of Jakob Nielsen's advice) and they tend to pay
with Amazon vouchers.
