This is a variation of "no cure, no pay" - and I have no problems
with this. But, if I'm taking the economic chances, I want to be able
to make the key design/development decisions. Alas, clients are
rarely willing to give away their authority to the degree required to
make this work. Someone (like the CEO's wife) invariably comes along
and demands I change the color of the links. I won't risk my
company's economy unless I have the right to say "go away" to the
bessermachers.

If anyone has a workable, practical, real-world solution, I'd be
interested in hearing about it.
