Hi all, I am looking for resources or guidance on conducting post-mortems.
I launched my first project at this job and intend to do a post-mortem and
realized I need some help.

I will do an internal team post-mortem as well as one with my clients.  We
are fairly informal here and have a close working relationship with the
clients and we will do more projects together.
 
Any websites, sample documents, tips or tricks you care to share with me? 
Anything from questions to ask to formats for conducting them.  I can
compile and post to the list or we can discuss - up to you.  

I'm doing some research on my own as well and am happy to share with anyone
interested.
