We use a product called SelectSurveyASP Advanced from Class Apps
(http://www.classapps.com).  It fairly comprehensive and easy to use.  My
only concerns with it have been with the export features.  You may need to
tweak the logic to get the data in a format that is really useful.

In general the application is very inexpensive, lightweight, and easy to
maintain.  The staff and developers are also very helpful.  Another great
thing about this application is that you are free to make any code
modifications you see fit and you are entitled to free upgrades for a year.
It's important to remember that the complexity in upgrading is directly
related to the complexity of changes you have made to the baseline code.
