A solution we recently came up with that fared extremely well in usability
testing was to use DHTML to create the pulldown menu. Clicking on an item
with sub-items displays a second list of options in the same pulldown menu.

For instance, the first view in your case might be an alphabetical list of
countries.  If the user selects US or Canada, the pulldown would then
display the list of states or provinces - again, in alpha order (assuming
regions aren't critical).  If the user selects another country, the pulldown
would close and the correct site page would display.

In our case, not-very-experienced users had to toggle between several views
using the pulldown - and were even more comfortable with this than we'd
hoped.
