At IconMedialab Germany, we would video only the screen. I felt strongly that having a video camera pointed at the user's face would disrupt the user's ability to focus on the tasks at hand. My research seemed to confirm this impression. In fact, we had some people who balked at being recorded at all (a fact that others have probably observed as well). Interestingly, though, perhaps there is a cultural issue involved.
I also tape-recorded interviews and usability tests when working on a multinational project. With tape recordings I didn't observe significant "drawback" of the subjects.
