[Sig-l] Revised Annual Report form

For those of you who have not submitted your annual SIG report, you can use  
the simplified one attached to this email.  If you have already  submitted the 
old form to meet the SIG-of-the Year deadline, the old form  is fine -- there 
is no need to re-do anything.
